SEMS is present at many special events around campus, and we may be available to staff your event. To request a SEMS team at your event, fill out the form below and submit it for instant review. Standby Requests should be submitted within 2 weeks of your event, and a SEMS representative will contact you within 2 weeks of submission.

Alternatively, you can print and complete the PDF file below, then return your completed form to a SEMS representative or send it via intercampus mail to Mail Code 2104, Department: Campus Health Services, Attn: SEMS.
